- Call LifeStream at (765) 759-1211 and ask for a “Medicaid Waiver Assisted Living Phone Interview.” The only purpose of this call is to schedule a face-to-face interview with one of their case workers.
- Make sure you have a list of current medications and health conditions when you make the call. Discuss your areas of need with the LifeStream representative.
- At the face-to-face meeting, which will take place where the applicant is currently living, please try to have a medical professional from the facility present. You and the medical professional will need to discuss your current health deficiencies as well as all medications being taken and areas where extra help is needed.
- Once the face-to-face interview is complete, the findings are sent to the state for final approval. Once approved, this is the “health” portion of approval. The entire process from your first call to approval usually takes about 30 to 45 days from the date of application.
- Things to remember: to go on the waiver, you must have “health” (LifeStream) approval and “financial” (Medicaid) approval.
- Once approved by both LifeStream and Medicaid, please let the Executive Director know. This will change your billing and what you pay each month.
